Health Information and Patient Records

Contact form: your name, email address, phone number, and the message you send.

Volunteer application: your name, email address, home and cell phone numbers, mailing address, date of birth, confirmation that you are at least 18, the volunteer roles that interest you, your availability, your employment status and occupation, previous volunteer experience, skills, languages, any accommodations you request, your responses to the background-related questions, your typed signature and the date, and the name and contact information of the reference you provide.

We use this information only to respond to you and to review and process your inquiry or application.

Information Collected Automatically

Like most websites, our web server records standard technical information when you visit — such as your IP address, browser and device type, the page that referred you, and the date and time of your visit. We use this information to keep the website secure, to diagnose problems, and to understand general usage.

Cookies

This website uses a small number of cookies:

Volunteer References and Clearances

If you apply to volunteer, you authorize us to contact the reference you name. Volunteer service also requires state and federal clearances — a Pennsylvania State Police criminal record check, a Pennsylvania child abuse history clearance, and, where applicable, an FBI fingerprint clearance. Those clearances are issued by the responsible government agencies and are handled according to their procedures and Pennsylvania law.

How Long We Keep Information

We keep form submissions and volunteer applications for as long as needed for the purpose for which they were submitted and to meet our recordkeeping, legal, and reporting obligations. You may ask us to delete information you submitted through this website by contacting us using the details below.

Security

This website is served over an encrypted (HTTPS) connection, and we take reasonable measures to protect the information we receive. However, no website or email transmission is completely secure, and we cannot guarantee the security of information sent to us over the internet.

Children’s Privacy
