Ann Silverman Community Health Clinic (“ASCHC,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains what information we collect through this website, how we use it, who we share it with, and the choices you have. It applies to this website only.

This Privacy Policy does not apply to the medical, dental, behavioral health, or social services records we maintain as a health care provider. Those records are protected under separate federal and state laws, and the way we handle them is described in the clinic’s Notice of Privacy Practices, which is available at the clinic.

Please do not send medical information, diagnoses, or other health details through the forms on this website or by email. These channels are not secure. If you need to discuss your care or your eligibility, please call us at 215-345-2260.

We collect the information you choose to give us through this website:

Contact form: your name, email address, phone number, and the message you send.

Volunteer application: your name, email address, home and cell phone numbers, mailing address, date of birth, confirmation that you are at least 18, the volunteer roles that interest you, your availability, your employment status and occupation, previous volunteer experience, skills, languages, any accommodations you request, your responses to the background-related questions, your typed signature and the date, and the name and contact information of the reference you provide.

We use this information only to respond to you and to review and process your inquiry or application.

Like most websites, our web server records standard technical information when you visit — such as your IP address, browser and device type, the page that referred you, and the date and time of your visit. We use this information to keep the website secure, to diagnose problems, and to understand general usage.

This website uses a small number of cookies:

• A language preference cookie that remembers whether you are viewing the English or the Spanish version of the site.
• Cookies that our forms may set to keep your session working while you fill them out.

We do not use advertising cookies, and we do not use Google Analytics, Meta/Facebook pixels, or similar tracking or profiling tools on this website. You can block or delete cookies through your browser settings, though some features may not work correctly if you do.

We do not collect or store credit card or bank account numbers on this website. Online donations are processed by PayPal and Venmo on their own websites. When you donate through one of those services, your payment information goes directly to that company and is governed by its privacy policy, not ours. Donations sent by mail or made by phone are handled directly by clinic staff.

Some pages include content hosted by other companies — for example videos from Vimeo, maps from Google Maps, and links to photo galleries on Google Photos. These services may collect information about your visit, including your IP address, and may set their own cookies. We do not control them.

This website also links to other organizations’ websites. This Privacy Policy does not apply to those sites, and we encourage you to review their privacy policies.

We do not sell, rent, or trade your personal information.

We share the information you submit only:

• with clinic staff and authorized volunteers who need it in order to respond to you or to review your application;
• with the service providers that operate this website and its email and form services, and only as needed for that purpose;
• when required by law, or to protect the rights, safety, or property of the clinic, our patients, our volunteers, or others.

If you apply to volunteer, you authorize us to contact the reference you name. Volunteer service also requires state and federal clearances — a Pennsylvania State Police criminal record check, a Pennsylvania child abuse history clearance, and, where applicable, an FBI fingerprint clearance. Those clearances are issued by the responsible government agencies and are handled according to their procedures and Pennsylvania law.

We keep form submissions and volunteer applications for as long as needed for the purpose for which they were submitted and to meet our recordkeeping, legal, and reporting obligations. You may ask us to delete information you submitted through this website by contacting us using the details below.

This website is served over an encrypted (HTTPS) connection, and we take reasonable measures to protect the information we receive. However, no website or email transmission is completely secure, and we cannot guarantee the security of information sent to us over the internet.

This website is intended for a general audience. It is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through this website. Parents and guardians are welcome to contact the clinic about services for their children.

You may contact us at any time to ask what information you submitted through this website, to correct it, or to ask us to delete it. You may also ask to be removed from any mailing list we maintain for clinic news and updates.
